Case Report on Congenital Muscular Torticollis

Abstract

Congenital muscular torticollis is an uncommon congenital musculoskeletal disorder that is characterized by sternocleidomastoid muscle shortening, lateral head tilt to one side, and contralateral of face and chin rotation.

Clinical Findings: A lateral head tilt to the left, with a facial deviation to one side.

Diagnostic Evaluation: CBC-Hb – 11.9 gram %, TLC- 7,600 / mm, HCT- 35.8.

MRI: The MRI revealed atlantoaxial instability in the form of displacement of the dens towards right; it also revealed that there was decreased bulk and kinking of left of left sternocleidomastoid. A diagnosis of congenital muscular torticollis was made.

Therapeutic Intervention: Surgical Intervention: Soft tissue reconstruction for congenital muscular torticollis. Post-operative intervention: 1unit RL@ 100ML/hr., 1unitNS@100ML/hr., Inj. Pantoprazole 40mg OD, Tab. Chymoral forte TDS 3days, Inj. Ceftriaxone 1gm IV BD 3day, Inj. Diclofenac Sodium 75gm IM 1day, Tab. Paracetamol 650mg TDS, Tab. Vitamin C 500mg BD.

Outcome: After operation, soft collars and braces are provided and movement therapy was given to the child. The child showed improvement.

Conclusion: The patient was admitted to orthopedic ward no- 33, AVBRH with known case of Congenital Muscular Torticollis and she had a complaint of deformity over left side of the neck. After getting appropriate treatment her condition was improved.
