Efficacy Of Modified Darn Repair with Mesh Rapair for Inguinal Hernia and Its Association with Early Complications

Abstract

Objective: To compare the efficacy of modified darn repair and mesh repair for inguinal hernia in terms of early complications and early recurrence.

Methodology: It was prospective, randomized – controlled trial, conducted at Department of Surgery, Chandka Medical College, Shaheed Mohtarma Benazir Bhutto Medical University, Larkana for 6 months (1st December 2015 to 31st May 2016). Total 78 patients with inguinal hernia were included and equally divided into group (A Mesh repair) and group (B Darn repair) groups. Inclusion criteria patients given informed written consent above 18-year age diagnosed clinically with inguinal hernia (inguino scrotal swilling with positive cough impulse). Exclusion Criteria strangulated hernia recurrent hernia obesity. During procedure, operating time was calculated. Postoperative early complications were recorded. Total expenses were calculated. Patients were discharged not earlier than 24 hours and were followed up after 1 week. At the end patients were contacted and early recurrence was checked. Descriptive statistics were calculated. Chi square test and t-test were applied for comparison considering p-value ≤0.05 as significant.

Results: The mean hospital stay in Group-A was 39.76±6.40 and 33.97±4.97 hours in Group-B. Mean pain score in Group-A was 4.23±2.69 and 3.64±2.97 in Group-B. Mean procedure time in group-A was 39.02±7.65 minutes and 36.38±5.16 minutes in Group-B. Mean cost o2 procedure in Group-A was 5794.87±1379.92 rupees and in Group-B it was 2884.61±702.09 rupees. Significant association of treatment groups was found with duration of hospital stay, duration to retain physical activities, and cost of procedure.

Conclusion: Darn repair proved superior to Mash repair in terms of patient’s early return to normal physical activity and less postoperative complications.
