Diagnostic Accuracy of Neutrophil to Lymphocyte Ratio in Pre Operative Diagnosis of Acute Appendicitis

Abstract

Objective: To determine diagnostic accuracy of neutrophil to lymphocytes ratio in diagnosis of acute appendicitis by taking histopathology as gold standard among suspected acute appendicitis patients.

Methods: It was a cross-sectional study conducted from Oct 2016 to Apr 2017. All patients who fulfilled the inclusion criteria and visited to emergency department of Surgery, Civil hospital Karachi were included in the study. Informed consent was taken from parents / guardian after explaining the procedure, risks and benefits of the study. CBC was advised and NLR was calculated. After appendectomy histopathology of each case was recorded. Patient's clinical presentation and histopathological findings were compared and analyzed to determine the diagnostic accuracy. All the collected information was entered into the predesigned proforma.

Results: Mean ± SD of age was 28.88±8.75 years. Out of 100 patients 70 (70%) were male and 30 (30%) were female. Diagnostic accuracy of NLR was 67% in diagnosis of acute appendicitis with sensitivity 69.62%, specificity, 57.14%, PPV 85.94% and NPV was found to 33% by using histopathology as gold standard.

Conclusion: It is to be concluded that patients NLR is a useful parameter to aid in the diagnosis of acute appendicitis and differentiate between simple and complicated appendicitis, and can be used as an adjunct to the clinical examination.
